1. Immunization Form (Required)
Every Union Graduate College student who takes more than 1.5 classes per term at any time must provide their immunization records on the required form. This is a NYS regulation but can cause some confusion for part-time graduate study students who take one course per term and then decide to take two courses in a particular term.

We strongly encourage part-time students who plan to take more than one graduate course per term at any point during their program, to submit an Immunization Form.
2. Permission to Release Educational Information to Union College (Required)
This form is generally completed at or before you register for your first graduate course(s). The purpose of this form is to allow Union Graduate College to release your ID to Union College in order to secure a student ID card and on-campus parking pass.
